- There is a $25 ID card replacement fee associated with this request.
- All requests must be accompanied by a valid (government-issued) piece of official identification. Acceptable identification includes: passport, birth certificate, driver’s licence, Certificate of Indian Status, permanent resident card, Canadian citizenship card, marriage certificate, provincial change of name certificate, court order proving a name change, foreign passport or Nexus card.
- Identification copies can be submitted to the Registrar's Office in person or by e-mail to transcripts@mta.ca.
